Join SCAN Online
Click the button below to go to the SCAN online enrollment page. There, you can enroll in SCAN by completing and submitting the online form.

A SCAN representative will contact you within 3 business days to confirm receipt of your online enrollment form.

SCAN Health Plan is a Medicare Advantage Organization with a Medicare Contract. Individuals must have both Part A and Part B to enroll. Benefits, formulary, pharmacy network, premium and/or co-payments may change on January 1, from year to year. You must continue to pay your Medicare Part B premium. Members may enroll in the plan only during specific times of the year. Limitations, copayments, and restrictions may apply. Contact SCAN Health Plan for more information.

Medicare Beneficiaries may also enroll in SCAN Health Plan through the CMS Medicare Online Enrollment Center located at http://www.medicare.gov.
