All members under both SCAN Health Plan Arizona and SCAN Long Term Care should receive a Member Identification (ID) card to show enrollment in our plans.
Members under SCAN Health Plan Arizona are issued a Member ID card from SCAN and members under SCAN Long Term Care should receive a Member ID card from Arizona Health Care Cost Containment System (AHCCCS).
Although an ID card does not guarantee that a Member is still eligible for services at the time of services. So, It is critical to verify insurance coverage and panel assignment to your practice (if applicable) before providing services.
Be sure to ask your patient for all insurance cards they might have, as they may have other insurance in addition to our plans.
Keep in mind when using this eligibility tool, only the co-payment amounts for that particular plan are listed. So, if a member is dually enrolled with both SCAN Health Plan Arizona Special Needs Plan (SNP) and an AHCCCS Long Term Care, the co-pays listed under the SNP plan would be covered by the Long Term Care Plan.
