SCAN is committed to safeguarding your personal information and maintaining your privacy. Throughout this Policy, SCAN will refer to information that personally identifies you or your accounts as "personal information." The purpose of this SCANHealthPlan.com Privacy Policy is to protect your personal information. This Policy applies to most of the information you provide via the SCANHealthPlan.com website.

Some information you provide may be entitled to greater protection than set forth in this Policy. "Protected health information", which is information that relates to the past, present, or future physical or mental health or condition of an individual; the provision of health care to an individual; or the past, present, or future payment for the provision of health care to an individual is protected under SCAN's Notice of Privacy Practices which can be obtained by contacting our Member Service Department at 1-888-540-7226, Monday – Friday, 8:00 a.m. to 8:00 p.m. TTY services for the hearing impaired call 1-800-367-8939.

How SCAN Protects Your Personal Information:
A. SCAN does not sell your personal information to anyone and SCAN uses commercially reasonable efforts to protect the security and confidentiality of your personal information.

B. SCAN only shares personal information with third parties in the following limited circumstances:

(i) SCAN discloses personal information to companies that help SCAN provide services you request via the website, for example, a company that mails out information on our services;

(ii) Sometimes SCAN enters into contracts with third parties so that they can assist SCAN in providing services, for example, companies that print and mail your account statements and inserts. SCAN's contracts with such companies prohibit them from using any of your personal information for their own purposes, and SCAN requires those third parties to maintain the confidentiality of the information SCAN has provided to them;

(iii) In limited circumstances, SCAN may disclose or report personal information where SCAN believes, in good faith, that disclosure is required or permitted by law; and

(iv) SCAN may share information with others if you give SCAN your consent.

C. SCAN collects personal information in the normal course of business in order to reply to requests for information and provide better service. For example, SCAN collects information related to grievances online. The information SCAN collects may include name, address, phone number, email address, your date of birth and member identification number.

D. SCAN may add to, delete or change the terms of this Policy from time to time, and will provide you with a notice of any changes via a link on the SCANHealthPlan.com home page. If SCAN ever intends to disclose personal information under terms that are more liberal than this Policy, SCAN will give you advance notice of the proposed change via the mailing address you provide to us and provide you with an opportunity to "opt-out" of such disclosure, if it is and as required by law.

E. SCAN uses commercially reasonable efforts to keep your personal information accurate and current and continually evaluates its efforts to protect personal information. If you need to make a change to your personal information, or you find an inaccuracy in your personal information, please immediately contact SCAN by calling us at 1-866-563-7321 so that we may promptly update our records.

F. SCAN uses commercially reasonable efforts to protect the confidentiality and security of your personal information. If SCAN hires a company to provide support services, that company is not allowed to use your personal information for their own purposes and SCAN contractually obligates that company to maintain the confidentiality of your personal information. Further, SCAN limits that company's use of your personal information to the performance of the specific service SCAN has requested. SCAN restricts access to personal information to its employees and agents for business purposes only. All employees handling personal information are required to safeguard such information. SCAN guards your personal information through reasonable physical, electronic, and procedural safeguards.

G. SCANHealthPlan.com is not intended for minors and SCAN does not intend to collect personal information from minors through its website. SCAN encourages parents to get involved with your children's online usage and to be aware of the activities in which they are participating. You should be aware that if your child voluntarily discloses his or her name, e-mail address, or other information on any web site, such disclosures may result in unsolicited messages.
