SCAN Long Term Care is part of the SCAN family, a non-profit group of health plans, whose mission is to provide quality care to meet the needs of the elderly and physically disabled. SCAN has been providing care to these members for nearly thirty years.

SCAN Long Term Care is proud to offer quality services to members who are enrolled in the Arizona Long Term Care Systems (ALTCS) program in Maricopa County.

  • Doctors - Choose from a network of over 2,600 primary and specialty care physicians developed specifically to meet your needs.
  • Case Manager "Your Partner In Healthcare" - Develop a personal relationship with a dedicated case manager whose goal is to work with you to keep you healthy and independent.
  • SCAN OnCallTM for Arizona - Get health information by phone from registered nurses, available FREE to you as a SCAN member, 24 hours a day, 7 days a week. Simply call, toll-free 1-(877)-582-7226.
  • SCAN Connections Resource Center - Come visit our community resource center to get health care information and to learn more about community services and resources. SCAN Connections is scheduled to be open in early 2007.
